Outreach Van to Support Families Struggling with Crisis

We provide essential supplies directly to individuals and families in crisis, including hygiene kits, food, water, clothing, and overdose prevention resources. This service addresses urgent survival needs while creating a safe entry point for engagement. By stabilizing immediate conditions, we increase the likelihood of individuals accepting ongoing support and services.

2. Mobile Intake & Case Management
Our team conducts real-time, on-site assessments to identify barriers to stability, including housing status, employment needs, and reentry challenges. Participants are immediately enrolled into coordinated case management services, ensuring no delay between outreach and support.

3. Housing Navigation & Placement Support
We connect individuals directly to housing opportunities through our network of landlords, transitional housing programs, and public housing pathways aligned with the U.S. Department of Housing and Urban Development. Our team provides hands-on assistance with applications, documentation, and placement coordination.
Outcome: Accelerated transition from homelessness to stable housing.

4. Employment & Job Readiness Connection
Participants receive immediate linkage to workforce development resources, including job readiness assessments, resume support, and referrals to employment partners. This service is designed to move individuals toward income stability as quickly as possible.

5. Transportation & Access Support
We remove one of the biggest barriers to success—lack of transportation—by providing direct rides and coordinated access to critical appointments, including housing interviews, job opportunities, treatment services, and court obligations.

6. Reentry Support Services
We work with individuals returning from incarceration to ensure they have a structured pathway to reintegration. Services include housing navigation, employment connection, and compliance support to reduce recidivism and promote long-term stability.
